Medical Social Services are available for patients who are on home health services through a contracted provider. This service is classified as a secondary service by Medicare and can only be initiated if the patient requires and is receiving one of the primary services: Skilled Nursing, Physical Therapy or Speech Therapy. The Medical Social Services must be prescribed by a physician and included in the Plan of Care. This service must be furnished by a Medical Social Worker and must be necessary to resolve emotional or social problems, which are an impediment to the effective treatment of the patient’s medical condition.
