Premature babies and high-risk infants are followed after being released from the hospitals that are designated as high-risk centers.

The pre-discharge visits and post-discharge visits are made according to referrals and any other follow-up that may be necessary in order to report the growth and development of the infant.

Investigative and referral visits are made for the Department of Children and Family Services and for the Division of Specialized Care for Children.

Referrals are also made to Child and Family Connections for further in-depth developmental testing.
